Update to the Preventive Health Care Checklist Form

The Preventive Care Checklist Form^© enables family physicians to provide comprehensive and evidence-based preventive health care to average risk adults. The forms contain recommendations reviewed by the Canadian Task Force on Preventive Health Care and other Canadian organizations. Recent updates include information about immunizations, preventive health measures and screening for cancers and chronic diseases.
