Fellowship in The College of Family Physicians of Canada (FCFP)

Fellows demonstrate a commitment to achieving and maintaining excellence across a range of competencies that characterize outstanding family physicians, as well as adherence to the Four Principles of Family Medicine. They are actively engaged with their practice, community, colleagues, and profession.

The College of Family Physicians of Canada (CFPC) awards the Fellowship designation (FCFP) to those who have been awarded Certification in the College of Family Physicians (CCFP) and who distinguish themselves through outstanding contribution to their communities, their colleagues, and the discipline of family medicine. Fellowship is considered a value of membership and will be bestowed if the member remains in good standing with the College.

The nomination period begins annually on the first business day of January and closes on the last day of March. All peer nominations and self-nominations must be completed and submitted within this time frame.


Eligibility

To begin the nomination process, it's important to first determine whether the nominee meets the eligibility requirements.

Use the nominee eligibility search tool to determine whether a CFPC member is eligible for nomination.
The search tool will identify whether the member is eligible by confirming that:
  • They are an active CFPC member in good standing
  • They have held the CCFP designation for a minimum of 10 years prior to the Fellowship nomination
  • They are not currently a Fellow
If a member is eligible their nomination must demonstrate that they also meet the following criteria to receive Fellowship in the College. The nominee must demonstrate:
  • A commitment to the broad range of competencies that characterize family physicians through activities related to at least four of the seven CanMEDS–Family Medicine Roles (Collaborator, Communicator, Family Medicine Expert, Health Advocate, Leader, Professional, Scholar)
  • Contributions to, or through, family medicine by upholding the Four Principles of Family Medicine, namely:
    • The family physician is a skilled clinician
    • Family medicine is a community-based discipline
    • The family physician is a resource to a defined practice population
    • The patient-physician relationship is central to the role of the family physician

Nomination Review Process

Nominations are reviewed by members of the three Nomination Committees – Reviews, Adjudications, and Appeals. Current Fellows are encouraged to join the Fellowship Council and participate in the Fellowship process through one of the committees.   • Each nomination will consist of a statement from the nominee and two letters from nominators who support the nomination, highlighting the nominee’s contributions to family medicine
  • Each nomination will be reviewed by three Review Committee members to determine whether or not it is successful
  • If the decision is unanimous, the nominee is notified of the determination
  • If the decision is not unanimous, the nomination—including reviewer feedback—will be given to a member of the Adjudication Committee to review and make a final determination
  • If a nomination is unsuccessful for any reason, the nominee may appeal the decision, in which case the Appeals Committee would review the nomination and any additional documentation to reach a final determination
  • Any unsuccessful nominees may be nominated again in a future year

Self-nominate

Members who are nominating themselves for Fellowship must use the Self-Nomination form.

All self-nominations require:
  • Submission of statements from two eligible nominators (one of which must come from a CFPC member in good standing) highlighting the nominee’s contributions to/through family medicine, as related to the Four Principles of Family Medicine
  • Completion of the nominee form identifying activities in at least four of the seven CanMEDS–Family Medicine Roles, demonstrating the broad range of work that characterizes outstanding family physicians
Note: Patients and family members are not eligible nominators. Student and Resident members are not eligible to serve as the CFPC member nominator but may provide the second supporting nomination letter.

Nomination reviews

The CFPC Fellowship Council will review all nominations and determine whether the member has successfully met the criteria. All determinations for Fellowship nominations will be communicated in July and successful nominees will receive their FCFP designation at Convocation at Family Medicine Forum (FMF) in November.
 

Provide a nomination letter for a CFPC member

Anyone nominating a CFPC member other than themselves should use the peer nomination form.

Nominators must provide:
  • Contact information for the nominee
  • A statement identifying the broad range of contributions the nominee has made, specifically related to the Four Principles of Family Medicine
Nominators who are members of the CFPC will have the opportunity to include a nominator statement on behalf of a second nominator. If a second nominator statement is not included, the nominee will be responsible for submitting one.

General Fellowship Questions


  • When will nominations be accepted?

    The nomination system opens annually on the first business day of the year and ends on the last day of March.

  • How can I submit a nomination?

    Nominations are made exclusively through the online system.

  • Who can nominate someone for Fellowship?

    Nominations may be submitted by a peer, a representative from a university, a CFPC Chapter, the CFPC National office, community official/leader, and/or other members of the community or professional organizations. Self-nomination is also acceptable.

    Patients and family members are not eligible to nominate, and cannot be involved in the nomination process. Additionally, for the purpose of nominations, Student and Resident CFPC members are not considered member nominators.

  • What are the maintenance requirements for Fellowship?

    Fellowship is a permanent designation as long as the member remains a CFPC member in good standing. There are no Mainpro+® maintenance requirements specifically for Fellowship.

  • What if I received my FCFP designation under one of the previous processes?

    If you achieved Fellowship under one of the previous processes, your designation will remain in place, and requires no action by you. Your FCFP will also now be a permanent designation, as long as you remain a CFPC member in good standing.

  • Is there an appeals process?

    Yes. If a nomination is unsuccessful, the nominee may appeal the decision through a process of providing additional information to support the nomination, for consideration by a member of the Appeals Committee. The decision of the Fellowship Appeals Committee is final and binding.

  • What is meant by “demonstration of significant contributions to, or through, family medicine”?

    Fellows should demonstrate a commitment to achieving and maintaining excellence across the broad range of competencies that characterize outstanding family physicians, and to upholding the Four Principles of Family Medicine, namely:
    • Clinical expertise
    • Serving as a community resource
    • Serving as a resource to a defined group of patients
    • Building and nurturing the relationships in practice
  • Are the contributions limited to the medical community?

    No. Committee and community work is not limited to the CFPC or the medical community. “Contribution” is defined broadly in respect to recognition.

  • What kinds of activities would be recognized as contributions to the community?

    Qualifying activities include:
    • Community or practice engagement
    • Teaching
    • Program development
    • Practice innovation
    • Leadership role(s)
    Broader definition examples include such activities as promoting health in the community through speaking to community/school groups, participating in coaching or mentoring opportunities in the community, and developing public health messaging for a clinic, hospital, or community.
 

The Fellowship Council

  
  • Who/what is the Fellowship Council?

    The Fellowship Council is a volunteer advisory body consisting of Fellows of the College who wish to take part in the Council or one of the committees (Reviews, Adjudication, or Appeals). The Fellowship Council is open to all FCFP designees in good standing and has the primary responsibility of reviewing and assessing nominations. The Council may also periodically be called upon to advise the College on the discipline of family medicine and the Canadian health care system. All members who hold the FCFP designation are invited to participate in The Fellowship Council.
