Progressing Team-Based Care in Alberta
Progressing Team-Based Care in Alberta

On behalf of the Alberta College of Family Physicians (ACFP) and in collaboration with the Coalition for Primary Health Care (CPHC), we are excited to share our summary video that captures the essence of the Team Primary Care Alberta (TPCA) forum held on October 5, 2024, in Calgary, Alberta, Canada.

The TPCA forum brought together primary care team members (family physicians, nurse practitioners, pharmacists, etc.), citizens, partners, and policy makers motivated to co-design, build, and commit to the supports and frameworks to optimize team-based primary care in Alberta’s health care system.

Join the Team-Based Primary Care Community of Practice to contribute to the conversation.
